Bi Describe the terms and conditions of your employment as set out in your contract of employment agreement. I am employed by Bupa Care Services.On the 14th January 2014 i received confirmation of a conditional offer of employment as a Care Assistant (Nursing and Residential) at Millview Nursing Home. The offer was subject to the satisfactory receipt of 2 written references , a medicheck form and a [DBS Disclosure/Disclosure Scotland] as i had worked recently in Humbie in Scotland.The Letter was signed by Neil Hooley the Interim Home Manager.The pay i was offered was £6.41 an hour and a 36 hour contract.The Pay is calculated on a weekday xhours at £6.41 weekend hours x £6.41 over my 36 hours an extra £1 an hour =£7.41 .Training is paid at £6.41 an hour .Pay is monthly. I started work on the 17th of February 2014 on a probationary contract until i was trained .My performance was assessed regularly to ensure i was carrying out my job to a satisfactory level.I recieved a letter from Bupa on the 14th November2014 thanking me for my High standards of care to the residents and confirmation of my completion of the probation period.I am expected to work additional hours which are necessary for efficient conduct.I have regular breaks of 15 minutes twicw a day and half an hour for my lunch .If i am working a half day i am allowed 20 minutes. My hours are personal to me and are set out in my terms and conditions. Lockers are provided in the staff room for sefeguarding our possesions .Flexible working hours are negotiable with the management. Reviews on performance are given annually and further training is always available for discusion. Private work must have approval from the manager , in case it conflicts with the interests from my work with Bupa.
